Client Service Coordinator

We need a friendly, outgoing, caring and resourceful individual to join the team and help us accomplish our many goals. This front desk position is primarily administrative support, but can be expanded or grow based on the candidate’s abilities, motivation, and passion. This is a full-time position supporting direct client services, coordinating projects and outreach, and engaging with the community and partner organizations. Currently, the projects include: conducting outreach and process Skagit County Rental and Utility Assistance applications, processing food assistance inquiries via our partnership with African Supermart and help with client check-ins.

Case Manager

The Case Manager is responsible for all client documentation creation and upkeep, financial records, medications as applicable, health and safety documentation, and all WAC and program requirements, assuring agency compliance in all areas associated with client documentation requirements in accordance with DSHS policies. Creates client IISP, PBSP, Protocols, release of information, and other required documentation for timeliness of completion. Assists with the transition of clients into and out of the program as needed.

Housing Coordinator

Under the Right of Way Initiative of Snohomish County, the Housing Coordinator is responsible for providing housing case management to unhoused individuals living on Rights of Way transition from homelessness to permanent housing. The Housing Coordinator is also responsible for daily program operations and reporting requirements.

Outreach Coordinator

The purpose of this outreach is to offer information, referral and direct supportive services to help individuals access needed community resources. Though the outreach may be conducted with a team member, the nature of this position is extremely independent, requiring excellent interpersonal relationship and communication skills, common sense regarding personal safety, creative thinking, multitasking, and conflict resolution skills. This position entails going out into the community to contact, develop rapport, and engage with individuals who are living outside. Outreach Care Coordinators are designated to particular neighborhood areas in order to maximize opportunities to connect with individuals living outside.

Substance Use Disorder Professional

The MH/COD Clinical Supervisor works under the supervision of the Program Director. The MH/COD Supervisor provides direct services to patients diagnosed with mental health or co-occurring disorders and provides clinical supervision to assigned staff
